Creating IDEAL Communities

What is Collective Impact?

Collective Impact brings people together, in a structured way, to achieve social change. Collective impact is a network of community members, organizations, and institutions who advance equity by learning together, aligning, and integrating their actions to achieve population and systems-level change.

Vision for Collective Impact:  People who experience disability and older adults have a wide array of inclusive, affordable housing options that they can easily access as demonstrated in the Triangle area

Collective Impact Objectives

  • OBJECTIVE #1

    Create IDEAL communities using an intentional-neighboring design and two scalable models.

  • OBJECTIVE #2

    Strengthen knowledge, understanding and engagement from a broad array of community members.

  • OBJECTIVE #3

    Strengthen self-determination and increase a sense of belonging among our target audiences living in IDEAL communities.

  • OBJECTIVE #4

    Develop and strengthen a Collective Impact approach.

The Cares Program at the UNC-CH School of Social Work, in partnership with North Carolina Money Follows the Person awarded HOPE NC a grant up to $150,000 a year for four years, with the potential for five years. This critical community impact grant, Building Capacity for Home and Community Based Services Through Collective Impact, will accelerate HOPE NC’s work to identify and develop opportunities to create inclusive communities where people of all ages and abilities engage with each other.

Collective Impact for Inclusive, Diverse, Engaging, Affordable and Lifelong Communities (Collective Impact for IDEAL) is building communities that share the values in our name. Communities that create a sense of belonging for all neighbors, not just adults with I/DD, other disabilities, and older adults.

Thank you to these community partners as we work together to make change across the state of North Carolina—together we will increase inclusive housing options for adults with intellectual and developmental disabilities and older adults.

North Carolina Inclusive Housing Coalition

As part of our collective impact efforts, we have pulled together 15+ groups (made up of parent advocates and non-profits) working to develop housing for adults with I/DD and older adults across the state of NC. This coalition meets regularly to share resources, problem-solve and advocate for state resources to address this growing need. Are you working on housing for these groups in NC?

We can’t do this alone.
