Durham, NC - North Street Community

An intentional, inclusive community:

  • North Street is a unique neighborhood designed to bring people of all abilities together. With support from HOPE NC, the community is strengthened by the presence of an onsite Community Facilitator who leads inclusive programming, fosters meaningful relationships among neighbors, and connects residents with essential resources.

Raleigh, NC - Grosvenor Gardens

An historic apartment building consisting of 50 apartments:

  • Grosvenor Gardens is a historic apartment building in Raleigh, home to 50 residents living in a welcoming, inclusive environment. Through a partnership with CASA and Alliance Health, 10 adults with intellectual and developmental disabilities (I/DD) live in apartments alongside neighbors without disabilities.

    An onsite Community Facilitator helps foster connection, support community engagement, and ensure all residents feel included and empowered in their day-to-day lives.

A mixed income neighborhood built by Habitat for Humanity:

Chapel Hill, NC - Weavers Grove (Coming 2026)

  • Coming in 2026, Weavers Grove is a new mixed-income neighborhood being developed by Habitat for Humanity in Chapel Hill. As part of this inclusive community, HOPE NC will purchase 10 duplexes to rent to adults with intellectual and developmental disabilities (I/DD), creating opportunities for independent living in a supportive environment.

    A dedicated Community Facilitator will help foster connection, inclusion, and engagement across the neighborhood — ensuring Weavers Grove is a place where everyone belongs.

A Community Built on Collaboration

Transforming housing, connection and inclusion-together.

What is the Collective Impact Project?

Funded by a four-year grant from NC Medicaid’s Money Follows the Person program and supported by UNC Cares, HOPE NC launched a statewide initiative in 2022 to expand inclusive housing options for adults with intellectual and developmental disabilities (I/DD). Partnering with leaders across housing, disability, aging, finance, and those with lived experience, HOPE NC developed the IDEAL Communities model—housing that is Inclusive, Diverse, Engaging, Affordable, and Lifelong—featuring a Community Facilitator in each setting. The initiative now focuses on piloting three projects in the Triangle, creating a replicable guide, and advocating for statewide policy and funding to support inclusive housing.
